If there is sound metal under the scale, descaling is the smaller job and buys you years, and the camera tells us which you have early on. If the wall has already rusted through in spots, taking the scale off only opens those holes, and relining is the honest answer.
Cleaning removes what sits loose in the pipe, like fat, paper and silt. Descaling removes what has bonded to the pipe itself, which water pressure won't touch and needs a flail on a shaft.
